Provide case management services to pregnant women and infants.


Family Case Management Services

  • Provide face to face services and ongoing assistance to families to remove barriers to receiving ongoing preventive health care services.
  • Provide education about the importance of child health including appropriate immunizations and screenings.
  • Refer the participant to appropriate providers within the community for services needed.
  • Ensure at least one home visit to assigned infants and pregnant women.
  • Update care plan after each contact.
  • Provide education and documentation on the Reproductive Life Plan and pregnancy education for all active women enrolled in Case Management.


  • Complete the required Family Case Management assessments.
  • Conduct and interpret screenings and provide referrals as needed.
    • Ages and Stages Questionnaire Social Emotional-2 (ASQ-SE2)
    • Ages and Stages Questionnaire (ASQ-3)
    • Edinburgh Postnatal Depression Scale Screening (EPDS)
    • Bright Smiles provide fluoride varnish on eligible clients

 Recordkeeping and Documentation

  • Maintain a tracking system for each family to ensure adequate coordination of care and timely home visits.
  • Maintain timely and accurate client files, case notes, and reporting records.
  • Document assessments, evaluations, and counselling in case notes.
  • Complete all required screens in the computer program.
  • Follow up and encourage clients to stay up to date with immunizations and well-child visits.

 Additional Duties and Responsibilities

  • Participate in staff meetings, conferences, training sessions and workshops as assigned.
  • Demonstrate familiarity with policies, performance standards and the agency's Mission, Vision and Core Values.
  • Exhibit a positive attitude and maintain friendly and respectful relations with staff, children and families.
  • Be a contributory team member in a positive/productive manner.
  • Maintain HIPAA and confidentiality policy.
  • Participate in WIC/FCM Outreach activities as needed.



  1. Master's degree or baccalaureate degree in a behavioral science, social science or health-related area; or a baccalaureate degree in any other area and one year of experience in child, family or community services; or an associate degree and two years experience in child, family or community services. 
  2. Home-visiting experience preferred.
  3. Must understand the social and economic challenges of the service area and be familiar with the community resources available for families.
  4. Strong knowledge and commitment to serving families of diverse social and economic backgrounds.
  5. Must be accepting, nonjudgmental, supportive, and compassionate when working with families.
  6. Knowledge on infant and child development preferred.
  7. Experience in working with families with multiple needs preferred.
  8. Must be able to work independently while adhering to program guidelines and recordkeeping requirements.
  9. Valid driver's license and access to an insured vehicle required.
  10. Positive, empathic approach and ability to communicate effectively with families.
  11. Possess excellent, highly developed listening skills and both written and oral communication skills.
  12. Must be detail-oriented, able to think critically and efficiently organized with the ability to maintain detailed records.
  13. Ability to work effectively in collaboration with diverse groups of people.
